Fault. Usually these types of situations are about circumstances. Neglect could be involved, but even that usually stems from some kind of miscommunication or failure to relate. If you wanted to get into the morality of it then you'd have to wonder how clear she was that the relationship is over, and even then her statement of 'he won't let me go' is an excuse on her part to use whatever comfort he provides while still being unfaithful. That's fucked up, depending on the circumstances, which only they know. That's why it's not a good idea to involve yourself in other people's intimate relationships.
