I'm not a compliance officer. I'm above that, lol.

I'm the guy the Compliance Officer's report too and determine penalty levels/citations.

But 6 years ago when I was a Compliance Officer, the strangest audit was probably this Factory UPSTATE NY who had just sustained a Work Place Fatality due to LOTO Violations and not having a required LOTO Program considering they operated Industrial Machines.

It was small, dark and smelt like a sewer. Like the company's factory was built over the septic system and the pipes burst, so sewage and fecal matter were leaking out from the bathroom floors into the factory floor. Pretty Gross.

Local Department of Health Agency's, New York State DEC and OSHA were involved Heavy LOL...

Put it this way, that business owner was not having a good day.

Needless to say, that company is still condemned until their penalties and citation are paid.
