My interpretation is Matthew Mitchell is not real the entire time, a manifestation of all the anxieties and skeletons from Eminem's past. In that sense, "Matthew Mitchell" still is rapping in the final verse.

My example is N.W.A. I love Ice Cube and Dr. Dre as solo artists and love a lot of rap from 1988 (when I was turning 1) but never quite got the appeal of N.W.A. That's because the biggest appeal of N.W.A. was they sounded completely different from what most people had heard. In the context of what came after, there's no shock value anymore.
